Important Information

•  You must complete and return the renewal form to us BEFORE the date of expiry together with your payment.

•  Cheques should be made payable to Banner Financial Services Group Limited.

•  If you are paying using a NON UK Bank cheque or Euro cheque please add an additional £28 to the value of the premium, as we will incur a bank charge to process your cheque.

•  Make sure that you disclose all medical conditions even if you have disclosed them previously so we can ensure our medical advisors assess you correctly.
